How is it that a Grand Lodge can refuse to recognize a PHA Grand Lodge if no formal request for recognition from a PHA Grand Lodge has been made? Or is your concern with Grand Lodge's that have not unilaterally recognized any and/or all PHA Grand Lodges?

I have no doubt that you have witnessed the refusal of black men. I have not yet witnessed the admission or refusal of a black man in my home lodge. However, I have been made aware that black men have been admitted to lodges that operate under my Grand Lodge, and that this was done so in accordance with that Grand Lodge's Constitution. As a matter of fact, and if memory correctly serves me, Grand Lodge representatives were in attendance to ensure that the Constitution was followed - to the letter.
